qmd is running but its health check cannot connect to `127.0.0.1:8181`

Recent qmd releases may bind localhost:8181 only on IPv6 (::1). A check hard-coded to IPv4 then fails even though the daemon and the configured MCP URL are healthy. Confirm with curl http://localhost:8181/health; it should return {"status":"ok",...}. The shared health check now uses localhost, matching the MCP server URL in packages/mcp-servers.txt.
